JUST THE FACTS:

Employee, was pushing, pulling and carrying a cart up and down stairs and also filing when she sustained injury to her left arm.

TREATMENT:

Employee sought treatment with a specialist who treated the injuries with medication, injections and surgery.

Employer and Insurer paid approximately $19,999.17 in medical expenses, including mileage, and $209.01 in Temporary Total Disability (sick pay). Permanent Partial Disability was assessed of 22.5% of the left elbow; equivalent to a gross payment of $21,560.51.

Attorney fees: 22.5% of $21,560.51 plus payment of out-of-pocket expenses of $1,024.99.
