JUST THE FACTS:

Employee was crushed when an electrical pallet jack malfunctioned and pushed him against a cardboard box baler.

TREATMENT:

Employee was airlifted to a medical facility in St. Louis and received emergency surgery for the injuries he sustained.

Employer and Insurer paid $183.76 in Temporary Total Disability (sick pay). Permanent Partial Disability was assessed of 25% of the body as a whole referable to the bladder; equivalent to a gross payment of $25,726.40.

Attorney fees: 25% of $25,726.40 plus payment of out-of-pocket expenses of $592.83.
