WORKERS’ COMPENSATION SETTLEMENT
Occupation: Tree Trimmer
JUST THE FACTS:
Employee sustained first injury to his left ankle due to a fall and second injury to his neck due to being hit with a piece of wood.
Treatment to Employee’s injury of ankle included X-rays, CT Scans, MRI’s leading to surgery of the ankle and subsequent of physical therapy, while Employee’s second injury was treated by Occupational Medicine Specialist who ordered physical therapy.
Employer paid approximately $34,000.00 in total medical expenses for the employee. Temporary Total Disability benefits (sick pay) was provided but amount was not determined at the time of settlement. Permanent Partial Disability was assessed of 25% of the left ankle at the 155 week level and 5% Permanent Partial Disability benefits of the body as a whole to the neck; equivalent to a sum payment of $25,738.23 for both injuries.
Attorney fees: 25% of $25,738.23 plus payment of out of pocket expenses of $949.90.
